All Star Activewear offers Net-30 credit terms for qualified corporate customers. In order to establish credit terms, please print and complete the following credit application. Once completed, fax or send the application by US Mail to All Star Activewear. Once your credit application has been received, a sales representative will contact you to set up your account.

This credit application can be used for both net-terms credit and pay-by-company-check credit.
